Ages and Stages Questionaire

You are invited to participate in the Ages and Stages Questionnaires (ASQ):

A free, Parent-completed, Child Monitoring System through Family Connections at Taylor Academy.

This fantastic resource for parents, purchased from Brookes Publishing Company and made available by FC, allows you to understand your baby and/or child's development.

This system includes a series of 17 questionnaires designed to identify infants and young children who show potential developmental problems.

The questionnaires are designed to be completed by parents when a child is 4, 6, 8, 12, 16, 18, 20, 22, 24, 27, 30, 33, 36, 42, 48, 54, and 60 months of age.

Each questionnaire has specific questions that can be answered by the parent, caretaker or nurse, which is used to determine whether the child is on target or needs further evaluation. Most people have no trouble understanding and completing the questionnaire.

Remember, the ASQ is not a diagnostic tool and does not determine if your child is gifted, has learning disabilities, medical conditions or emotional disorders.

Each questionnaire takes approximately 10-30 minutes to complete. Scoring is as easy as completing the questions. Five areas covered on each age specific questionnaire are:

1. Communication

2. Gross motor

3. Fine motor

4. Problem-solving

5. Personal-social

As you play with your child, you are asked to observe the way that your child completes certain activities. You check "yes" if your child performs the activity regularly; "sometimes" if your child does it only occasionally; "not yet" if your child does not do the activity yet.

Once you complete the questionnaire, return it to FC and it will be scored by a staff person. FC will send you a letter with the results, complete with appropriate educational activities you may enjoy doing with your child. Please call FC with any questions about the ASQ program.
